The IU School of Optometry notified 757 patients that a computer server containing their personal health information was visible online for nearly a month between August and September 2011, according to an IU press release.
The server was exposed Aug. 12 and was shut dow Sept. 9. The school notified patients on Sept. 30.
The school was required by law to report the incident and launch an investigation.
